Announcement About Student Identity Cards

ANNOUNCEMENT ON STUDENT AND STAFF ID CARDS
"Institutional Identity and Visitor Card Directive", which was prepared in order to determine the procedures and principles regarding the issuance of the identity cards of the students of our university who have lost, broken, stolen, name-surname change, magnetic field disorder or a change by court decision and who want to change it voluntarily. and entered into force with the Senate Decision No. 2021/19-04.

In this context;

Following the steps of the Student Identity Card Printing Work Flow Chart below, in case of name-surname change, magnetic field disturbance and court decision, fill out the FRM-0802 form free of charge; In case of breakage, loss, theft and willingness to change, student ID cards will be renewed by filling out the FRM-0803 form.
Our students are required to apply to the student affairs of the academic unit they are registered with with the following documents.
